To register for our online services you will need to complete this form and then visit the practice, bringing with you two forms of identification. One of these items should include your photograph, and one should show proof of address.

We will then issue you a username and password.

Once you are registered you will be able to use the service to:

• Order your repeat prescriptions
• Make an appointment
• Cancel an appointment
• Change your contact details
• Review your medications and known allergies

You can still contact the practice by phone or in person. This is just another option, which an increasing numbers of patients have found more convenient and saves them time. More information, including “how to” leaflets and videos, are available at www.nhs.uk/gponlineservices

To sign up, please complete our online form, and visit the practice with proof of photographic ID (passport, driving licence) and proof of your address (e.g. bank statement).
